We are committed to your health education and want to provide a team approach to your healthcare needs. Before arriving for your initial visit, we want to provide you with some information that will help make this visit more efficient and respectful of your time.

• We request you complete your demographic and health history information prior to your appointment. You will receive a link to Qure4u which will navigate you through the check-in process. This can significantly reduce your wait time. If you are unable to complete this process online, please arrive 20 minutes prior to your appointment time to allow sufficient time for completion.
• Please bring a valid photo ID and your current insurance card(s).
• Please be prepared to pay any copays, remaining deductibles, or co-insurance at the time of service.
